    Submit our designated request form along with identity verification documents (for example, a copy of a driver's license or passport). If a representative files on your behalf, additional authorization documents are required.

  2. Fees

    For notice of purpose and disclosure requests, a fee of JPY 1,000 per request may apply. Payment method details will be provided by our staff.

  3. Response method

    We respond using your requested disclosure method after identity verification. If a request cannot be fulfilled, we will notify you with the reason.
